Overview
When Empathia came to us, they had no presence in AI search. Their visibility score was at zero, which meant they weren’t showing up in ChatGPT, Gemini, or Perplexity results when people asked about their category.
Empathia already had a strong foundation: a great product, powerful customer testimonials, and compelling case studies that should have built credibility. Still, none of it was being credited in AI-driven answers. Frevana started by running an in-depth AEO audit that revealed missing or misconfigured LLMs.txt and Robots.txt files. Then, we helped them build a clean, AI-citable launch page with structured customer reviews. We also set up a visibility dashboard so they could track exactly how often they were surfacing in top AI answers. Within just two weeks, their visibility score rose to 4%, and in the first week of starting with Frevana, they gained a paid conversion directly from ChatGPT. What began as complete invisibility turned into their first proof that AI search could drive real business outcomes.

The Story
Empathia is an AI-powered healthcare technology startup, serving as a personalized clinical assistant for physicians. The system acts as a medical scribe, allowing physicians to save hours each day on charting and admin work; Empathia provides chart prep, visit recordings, customized patient notes and instructions, customized billing codes, and a means of transferring records securely. Essentially, Empathia automates all of the admin work for physicians within one system.
Empathia’s content wasn’t optimized for AI systems to comprehend, which resulted in zero visibility in AI-generated answers—and subsequently very minimal online impact. Additionally, their domain sites were missing files from LLMs.txt and Robots.txt, meaning neither AI engines nor search engines had the key to their content door; ultimately, the site offered no clear entry point for the engines to learn. For a team trying to break into a competitive healthcare market, this was a serious setback.
The Turning Point
Frevana’s first step was running a comprehensive and in-depth AEO report that showed the team exactly where they were positioned and how visibility is measured in AI search—the visibility score is determined by how frequently a brand appears in the top list of AI search answers across various product/service related queries.
We then moved quickly to identify the problem (missing LLMs.txt and Robots.txt) and fix the foundation. Frevana provided an optimized data structure to allow AI platforms to read and trust their site. We created an enhanced launch page, presenting their product details in a way that triggers most questions asked by consumers on AI platforms. Finally, we created a visibility dashboard for the team to track their progress, as well as monitor their competitors’ performance.
The Outcome
Within two weeks, Empathia’s visibility score rose from 0% to 4%. That may sound small, but for them it was a breakthrough: it was their first quantifiable step into AI visibility. Furthermore, in the very first week of launching their landing page, they gained a paid conversion that came directly from a ChatGPT result recommendation. Perhaps most notably, Frevana helped them increase their organic traffic four times over in only two months. Empathia has successfully broken into the world of AEO, and seen direct improvements because of it.
Moreover, their new AEO visibility dashboard continues to help them project future outcomes, pointing towards a 70% visibility score in less than one fiscal quarter. By tracking frequency over time, they’ll see opportunities to boost their appearance in queries and eventually dominate the AI visibility in their product category.
The Future
Empathia has committed to a working partnership with Frevana, continuously building their presence for AI discovery. We will continue refining their landing pages, optimizing their domains, and expanding their content in ways that work for both readers and AI engines.
At Frevana, we cherished this opportunity as proof of a larger trend: the diminishing of endless SEO operations, and the rise of building structured, citation-optimized content for AI search results. We believe AEO will define the future of discoverability, and cases like this show why. Empathia is a firsthand example of how quickly the right approach can turn invisibility into visibility, and visibility into real business growth.
